A 49-year-old patient presents with forefoot pain, predominantly in the second toe of the left foot, associated with a progressive limitation in wearing closed shoes and carrying out everyday activities.

He reports several years of symptoms, with a progressive increase in severity, although he had not previously attended a specialist centre for percutaneous surgery or microsurgery due to fear of the procedure.

After clinical assessment and completion of the preoperative work-up, with favourable results, comprehensive surgical treatment of the forefoot is indicated.

The procedure is performed using a microsurgical technique, addressing correction of the associated hallux valgus and the claw toe deformity, with the aim of restoring function and improving forefoot alignment.

The surgery is carried out under local anaesthesia, and the patient is able to walk immediately after the procedure.

The postoperative course is favourable, with regular dressing changes and good tissue healing, with no notable incidents.

During follow-up, the patient shows progressive improvement in pain and function, reporting that he can wear shoes normally again\u2014something that had previously been very limited due to his symptoms.
