Clinical case — Bilateral hallux valgus (bunions)

A 64-year-old patient who comes to the clinic from outside the Community of Madrid due to pain in both feet, predominantly in the left, with significant limitation when walking for prolonged activities.

On clinical examination, bilateral hallux valgus is observed, with greater deformity and symptoms in the left foot, associated with forefoot overload.

Preoperative complementary tests are requested, including X-rays and routine blood work, which show good bone condition and no contraindications for surgery.

Given the distance from her home to the clinic and after an individualized assessment of the case, it is decided to perform bilateral surgical treatment in a single surgical session.

Hallux valgus surgery is performed using the MIS (minimally invasive surgery) technique, with millimetric incisions that are virtually imperceptible after healing, together with toe correction and soft-tissue release.

In the immediate postoperative period, the patient walks out of the operating room under her own power at discharge. She experiences mild pain during the first 48 hours, with discomfort controlled during the first two weeks without significantly interfering with her daily activity.

Wound care progresses favorably, with good healing and correct progressive alignment of the forefoot.

A biomechanical assessment and custom insole fabrication are prescribed for daily use, especially aimed at physical activity and preventing overuse.

The patient is discharged 5 weeks after surgery with a satisfactory recovery and returns home by plane without incident.
