Clinical Case — Unilateral Flat Foot in Adolescent Patient

A 15-year-old patient attends the clinic accompanied by his legal guardian, reporting discomfort and a sensation of fatigue in the right foot after intense physical activity.

Clinical examination reveals right unilateral flat foot, with the contralateral foot showing normal morphology and function, without pathological findings.

A complete biomechanical study and various functional tests are performed to quantify the degree of involvement and its impact on gait dynamics.

As part of the preoperative study, complementary tests are requested, including magnetic resonance imaging, X-rays, and presurgical blood work, with results within normal limits for surgery.

Following comprehensive evaluation of the case, surgical treatment is decided via microsurgery with soft tissue release and placement of an intraosseous implant in the right foot for corrective purposes. The implant is permanent and does not require subsequent removal.

The postoperative period progresses favorably. The patient begins progressive weight-bearing from day one, as tolerated, and performs assisted ambulation with a Walker boot. At 4 weeks, immobilization is removed and progression toward normal gait begins.

Custom insoles are also made to improve comfort and optimize weight-bearing biomechanics.

The outcome is satisfactory, allowing normal daily activities and progressive return to sports activity, including impact sports, without significant limitations.

The patient is discharged after follow-up without complications and has not reported symptoms or new episodes of pain since the procedure.
